KUALA LUMPUR: The Transport Ministry will set up two special task forces to investigate recent transportation incidents, following a post-cabinet meeting today.
The first task force will examine the container explosion at the Port of Tanjung Pelepas (PTP) on Nov 7, which occurred on board the MV Kyparissia during unloading operations and resulted in three deaths and three injuries.
Investigators will review operational procedures, safety compliance, and the handling of hazardous cargo.
The second task force will probe the Nov 8 incident at Butterworth Railway Station, where an empty commuter train overran the tracks at Platform 4.
The team will assess the cause of the overrun and recommend measures to prevent a recurrence.
In a statement today, Transport Minister Anthony Loke said the task forces are intended to ensure transparent, impartial investigations and to strengthen safety and operational integrity in the nation's transport sector.
He also directed the Land Public Transport Agency (Apad) to investigate the train driver and take appropriate follow-up action.
MOT said that user safety remains a top priority and urged all operators to strictly comply with safety procedures while carrying out continuous monitoring and maintenance. Updates on the investigations will be provided as findings emerge.
The meeting also addressed the rainwater leakage at Kuala Lumpur International Airport (KLIA) Terminal 1 last week.
The incident occurred during waterproofing works on the terminal's main roof by contractor Teras Budi Resources Sdn Bhd, under a RM5.27 million contract running from April 15, 2025, to April 14, 2026.
Loke said that Malaysia Airports Holdings Berhad (MAHB) has issued a stop-work order to allow an internal investigation and immediate corrective actions.
